Help Improve Teen Mental Health in California!

The California Department of Public Health’s Office of Health Equity is conducting an important confidential survey to better understand how young people experience stress and access mental health resources. Your feedback will help shape future programs that support teens across the state.

Why Participate?
  • Quick & Easy: The survey takes about 10 minutes to complete.
  • Completely Confidential: No personally identifiable information will be collected.
  • Earn a Gift Card! If your child qualifies and completes the survey, they will receive a Visa, Amazon, or Starbucks gift card as a thank-you for their time. You may also choose Uber Eats, Chipotle, Taco Bell and several others.

What’s Next?

Before your child can participate, we need your permission. Please take a moment to:
  • Answer a few brief questions.
  • Review important information about the survey.
  • Provide consent for your child to participate.

Once you give permission, your 13- to 17-year-old will be able to complete the survey either on this device or using a unique link on their own device.

This survey is by invitation only and not open to the general public—unauthorized social media posts were not approved.
